Office Policies

The office is a fee for service Oral Medicine practice. New patient visits depend on time, and are typically 45 – 60 minutes with the fee charged based upon the time required and any procedures performed. Additional charges may be made for special tests or additional time. Follow-up visits are typically 15 – 30 minutes, with fees depending upon time.

Payment is expected at the time of the visit and can be paid by cash, Visa, or MasterCard, or check. We will provide the information you need to submit forms for insurance reimbursement that will include the codes you need in order for you to submit. Most of our billing will be eligible for medical insurance, however some insurers may not cover all procedures that you may require. Some procedures and visits may be reimbursed with dental insurance. We require 48 hours notice for cancellation of the appointment without charge. A consult note will be sent to your doctor or dentist following the visit.  Please bring with you any prior records including medical or dental records, x rays, models and appliances you may have.

Financial

For Locations in the United States: 

There are two financial components to each of your visits.

1.  The first is the professional fee for consulting with the doctor. Please be advised that payment IN FULL for this consultation and services rendered is expected at the time of each visit. Payment may be made in cash or by credit card (VISA, MasterCard, Discover). If you have dental or medical insurance and would like to submit a claim for reimbursement to your insurance company, you will receive a receipt with the procedures, codes, and fees that you may send to your insurance carrier. While we will provide the information you need to submit, fulfilling your insurance carrier’s requirements is YOUR RESPONSIBILITY. Please note that Medicare will not reimburse you for this visit, and  HMO, PPO may not reimiburse you for this visit.

2. At the time of your visit, you will be asked to provide medical insurance information. This information is collected as part of your record, and may facilitate reimbursement to you for any requested medical laboratory or x-ray tests if orderd.   

 

For British Columbia:

Oral medicine services may be covered as a benefit of your dental insurance.  The office will provide the information you need  to submit in order to seek reimbursement.  Medical laboratory testing and x-rays are covered by the medical plan.
